About the position

The Case Manager 3 position within the Early Intervention Unit at the Arizona Department of Economic Security focuses on providing support and services to individuals with developmental disabilities and their families. The role involves conducting home visits, facilitating Individualized Family Service Plan (IFSP) meetings, and collaborating with various agencies to ensure the needs of children and families are met. The position emphasizes community engagement, advocacy, and the development of member service plans to enhance the quality of life for Arizonans.

Responsibilities

  • Conduct home visits to meet with individuals and families, developing relationships and understanding their dynamics and needs.
  • Facilitate multi-disciplinary Individualized Family Service Plan (IFSP) meetings at least every six months for children with developmental disabilities.
  • Educate parents about their rights and responsibilities, and provide advocacy skills training.
  • Assist families in the grieving process when applicable.
  • Collaborate with the Department of Child Safety (DCS) to meet the needs of children in the DCS system.
  • Regularly assess each child's progress and make appropriate referrals to community agencies.

Requirements

  • A minimum of a Bachelor's degree in Early Childhood, Early Child Special Education, Family Studies, or a closely related field such as Psychology, Social Work, Elementary Education, or Sociology.
  • One year of experience working with people with developmental disabilities is preferred.
  • Possession of a valid driver's license and ability to maintain acceptable driving records.
  • Valid Level One fingerprint clearance card to work with children and vulnerable adults.

Nice-to-haves

  • Experience in case management and knowledge of HIPAA regulations.
  • Skills in conflict resolution and effective communication.
