About The Position

The Field Case Manager at UnitedHealth Group is responsible for managing a panel of members with complex behavioral health needs, including mental health and substance use disorders. This role involves care coordination, advocating for patients, and ensuring they receive appropriate services to meet their medical, behavioral, and socioeconomic needs. The position requires significant fieldwork, with approximately 80% of the time spent visiting members in their homes or long-term care facilities, and offers the flexibility to work remotely within Florida.

Requirements

• 4-year degree plus 2+ years of relevant experience or 6+ years of relevant social service or case management experience without a degree.
• 1+ years of experience with MS Office, including Word, Excel, and Outlook.
• Access to reliable transportation and ability to travel within assigned territory.

• Experience serving individuals with co-occurring disorders.
• Background in managing populations with complex medical or behavioral needs.

• Bilingual Spanish.

Responsibilities

• Assess, plan, and implement individualized care strategies for patients.
• Identify and initiate referrals for social service programs, including financial and community support services.
• Manage the care plan throughout the continuum of care as a single point of contact.
• Communicate health-related information to stakeholders to ensure quality coordinated care.
• Advocate for patients and families to ensure their needs and choices are represented.
