About the position

The Case Manager position at the Community Correctional Facility involves providing counseling and casework services to reentrants and parolees facing behavioral and adjustment issues. The role focuses on facilitating institutional adjustment, offering support for personal and family problems, and guiding individuals towards successful community reintegration through various resources and referrals.

Responsibilities

  • Interview parolees to facilitate their institutional adjustment and provide encouragement.
  • Assist with personal and family problems, conflict resolution, and other concerns within the facility and community.
  • Provide referrals and guidance for developing social, educational, and vocational skills necessary for positive re-entry.
  • Aid in obtaining information regarding employment opportunities and community resources.
  • Assist in obtaining Medical and Food assistance services as needed.
  • Start or complete SSI applications for qualified individuals.
  • Develop and maintain necessary paperwork, reports, and documentation for managing caseloads.
  • Foster positive relationships between the facility, other departments, and community agencies for effective case management.
  • Confer with the Director, Lead Case Manager, and RD about a parolee's adjustment and progress.
  • Utilize assessment scores to determine recommended programming and maintain documentation regarding various evaluations and planning.
  • Coordinate treatment objectives using a multi-disciplinary approach to support effective resource utilization.
  • Facilitate compliance and behavioral change through the implementation of infractions and behavior contracts.
  • Participate in parole planning and make recommendations regarding re-entry services.
  • Secure forms of identification and assist with community reintegration.
  • Set up counseling appointment dates and ensure attendance.
  • Perform other related duties as assigned.

Requirements

  • Effective verbal and written communication skills.
  • Proficiency in Word, Excel, and Outlook is a must.
  • Experience with online applications for Food Stamps, Medical assistance, and SSI applications is a plus.
  • Ability to maintain regular and reliable attendance.
  • Experience in conducting new intake paperwork and orientations for new parolees.
  • Ability to enter daily/weekly case notes and maintain logs for caseload.

Nice-to-haves

  • Experience with community liaison services.
  • Ability to analyze and interpret information from Parole and DOC.

Benefits

  • Starting rate of $19.00, negotiable based on experience.
