Case Manager Aide

About the position

The Mercy House Case Manager Aide at Catholic Charities is responsible for providing direct services to clients who do not qualify for assistance in other agency programs. This role focuses on improving the social functioning and well-being of clients through social services and assistance, including counseling and resource referrals.

Responsibilities

• Interview clients individually to assess their situations, capabilities, and problems.
• Develop and review service plans in consultation with clients and perform follow-ups.
• Refer clients to community resources for services such as job placement, debt counseling, legal aid, housing, medical treatment, or financial assistance.
• Counsel clients regarding issues including mental health, poverty, unemployment, substance abuse, physical abuse, rehabilitation, social adjustment, childcare, or medical care.
• Collect supplementary information needed to assist clients.
• Work cooperatively with all external and internal stakeholders.
• Achieve individual productivity requirements and contribute to program productivity objectives.
• Complete and update case records for each client in accordance with agency guidelines.
• Represent the program in meetings, public forums, and workshops.
• Network with community providers regarding program services and client referrals.
• Prepare reports as required by the agency and funder.

Requirements

• Master of Social Work (MSW) or Bachelor of Social Work (BSW) degree or a related degree in social services.
• 3 years of experience as a Case Manager.
• Strong leadership and networking skills.
• Ability to work with the homeless or at risk for homelessness population.
• Excellent communication and listening skills.
