About the position

Collaboratively work as part of a multidisciplinary team to outreach, engage and provide follow up service navigation for individuals that have been assessed by the Alternative Response Team (ART). Provide client advocacy and assist individuals with accessing all levels of service delivery & community resources they need to recover. Provide a full range of crisis services which divert individuals experiencing a mental health crisis from inpatient hospitalization, jail, homelessness or emergency departments.

Responsibilities

  • Outreach and engage individuals assessed by the Alternative Response Team (ART).
  • Provide follow-up service navigation for clients.
  • Advocate for clients and assist them in accessing community resources.
  • Deliver a full range of crisis services to divert individuals from hospitalization, jail, or emergency departments.

Requirements

  • 6+ months of experience working with SMI clients required.
  • For Case Manager II: High school graduate or equivalent + 36 months experience in behavioral health OR 60 hours college credit + 12 months of behavioral health experience.
  • For Case Manager III: Bachelor's Degree in any field + 6 months experience in a behavioral health field required.
  • For Case Manager IV: Bachelor's degree in social science field required and Case Management II certification or obtain by taking the first available Case Management Certification course after hire.
  • Strong commitment to the rights and abilities of individuals served to live in community residences.
  • Skills to establish supportive trusting relationships with persons with mental illnesses and/or substance abuse issues.
  • Valid Oklahoma Driver License and satisfactory driving record.

Nice-to-haves

  • Experience in a Certified Community Behavioral Health Clinic (CCBHC) model of care.
  • Knowledge of community mental health and substance use services.
