About the position

The Case Manager for Adult Protective Services (APS) is a part-time, temporary role focused on providing essential case management services to functionally impaired seniors and adults with disabilities. The position aims to assist clients in obtaining necessary services to maintain optimal functioning, working closely with various support systems and agencies. The role emphasizes collaboration, advocacy, and the integration of mental health services as needed.

Responsibilities

  • Conduct intake and assessment of clients.
  • Complete and update care plans as provided by APS.
  • Coordinate among service providers and follow up on client progress.
  • Monitor, reassess, and facilitate discharge and transition of care within six months.
  • Advocate with outside agencies to secure benefits, housing, and resources for clients.
  • Participate in case review and consultation with supervisors and other case managers.
  • Engage in relevant training and collaborative efforts within the agency.

Requirements

  • Minimum of 2 years of experience working with older adults.
  • Strong collaborative skills with team members and community partners.
  • Solution-focused, fast-paced, person-centered approach.
  • Knowledge of Alameda County community resources.
  • Experience working with Adult Protective Services (APS).
  • Understanding of human behavior and performance.
  • Social perceptiveness skills to understand others' reactions.
  • Ability to communicate effectively in both spoken and written forms.
  • Anticipatory problem-solving skills.
  • Ability to work as a team player with a welcoming attitude.
