About the position

The Case Manager/Assistant Youth Coordinator at Chagrin Falls Park Community Center is a full-time position focused on providing essential case management services to individuals and families, particularly youth. This role is pivotal in fostering social, emotional, and educational growth among participating youth through various supportive services. The Case Manager will conduct assessments, provide age-appropriate case management, and assist families in navigating interactions with schools, healthcare providers, and social services. The position requires maintaining confidential client records and reporting outcome measures and demographics as mandated by funding requirements. In addition to case management, the role involves building and maintaining relationships with community social agencies to leverage resources for clients in need. The Case Manager will create a supportive, trauma-informed environment for families and will be responsible for recruiting and training volunteer tutors for students in grades K-3, managing classroom behaviors, and pairing students with tutors for optimal learning outcomes. Communication with the Kenston School District's administration and teachers regarding student progress is also a key responsibility. The Case Manager will participate in team planning and program goal achievements, utilize Microsoft Office for documentation, attend assigned meetings and trainings, and comply with agency policies and procedures, as well as federal and state regulations. Other tasks may be assigned as needed, making this a dynamic and impactful role within the community center.

Responsibilities

  • Implements or coordinates supportive services that foster social, emotional and educational growth.
  • Conduct assessments and provides age-appropriate case management.
  • Provides referrals and assists families in their interactions with schools, health care providers, and social services as needed.
  • Reports outcome measures and demographics as required.
  • Maintains confidential client records including up to date data entry as required by the funder.
  • Maintains relationships with community social agencies to capitalize on resources on behalf of the Center and clients in need of assistance.
  • Provides a supportive, trauma-informed environment to families.
  • Recruits and trains volunteer tutors during school year for grades K-3 and manages classroom behaviors.
  • Pairs students and tutors for optimal learning.
  • Communicates with Kenston School District's administration and teachers concerning student progress.
  • Organizes and maintains library.
  • Participates in team planning and program goal achievements.
  • Uses Microsoft office functions, e-mail, calendar and other data methods needed for documentation.
  • Attends meetings and trainings as assigned.
  • Complies with agency policies and procedures, COA regulations, federal and state requirements, and educational/certification/registry requirements.
  • Completes other tasks as assigned.

Requirements

  • Knowledge and ability to establish client relationships with professional boundaries.
  • Willingness to learn client-centered model and conflict resolution; ability to manage crisis situations.
  • Ability and understanding of case management record keeping, ability to communicate with clients and build trust, strong overall communication skills, both oral and written.
  • Ability to model open communication, fair and equitable treatment and conflict management skills.
  • Knowledge and ability to use Microsoft Office suite for recording information.
  • Mandatory CPR and First Aid Certifications.
  • Valid Ohio Driver's License and ability to meet agency requirements for driving insurability.
  • Must be able to meet agency requirements for criminal background check (Ohio Bureau of Criminal Identification & Investigation).
  • Degree in Social Work, Counseling, Psychology, Human Services or related degree preferred. High school diploma/GED required.
  • Experience working with populations that are low income and work with children in educational setting preferred.
