The Case Manager Assistant plays a crucial role in supporting Case Managers by ensuring that case management services, communications, and documentation are processed efficiently. This position serves as a liaison between patients, their families, and hospital staff, focusing on enhancing patient satisfaction and the quality of services provided.

  • Provide support to Case Managers in processing case management services.
  • Ensure effective communication between patients, families, and hospital staff.
  • Assist in maintaining appropriate documentation related to case management.
  • Act as a liaison to enhance patient satisfaction and experience.
  • Minimum of two (2) years customer service experience in a healthcare or related setting.
  • High School Diploma, GED Equivalent, or Certificate; Associate's degree in related field preferred.
  • Basic knowledge of computer systems.
  • Previous healthcare experience with discharge planning activities.
  • Ability to accurately document in medical records.
  • Effective oral and written communication skills.
  • Ability to establish and maintain effective working relationships.
  • Demonstrated initiative and self-direction.
  • Computer, keyboard, and data input skills.
  • Knowledge of Windows software.
