The Case Manager Assistant plays a crucial role in supporting Advocates and clients in navigating complex needs and accessing necessary resources. This position involves assistance with assessing client needs, developing personalized care plans, and coordinating services to promote overall well-being and stability. The ideal candidate will possess strong interpersonal skills and a deep understanding of community resources.

Responsibilities

• Conduct comprehensive assessments to identify client needs and goals.
• Develop, implement, and monitor individualized service plans.
• Facilitate access to social services, including Section 8 housing assistance and workforce development programs.
• Provide crisis management support to clients experiencing immediate challenges.
• Utilize motivational interviewing techniques to encourage client engagement and self-sufficiency.
• Collaborate with multidisciplinary teams, including social workers, addiction counselors, and behavioral health professionals.
• Maintain accurate documentation of client interactions, progress, and outcomes.
• Advocate for clients' rights and needs within various systems.
