About the position

The Case Manager position involves providing clinically indicated case management services to children, adolescents, and adult consumers and their families. The role includes extensive outreach and marketing activities to engage individuals and communities, ensuring continuity of care and addressing service needs through referrals and linkages to appropriate agencies.

Responsibilities

• Provides outreach, coordination, advocacy, and linkage services for consumers and their families.
• Engages with difficult-to-reach consumers and follows up on missed appointments.
• Assists consumers in meeting concrete service needs and makes referrals to appropriate agencies.
• Accompanies consumers to community appointments such as medical and educational.
• Identifies opportunities for community outreach and provides presentations on services offered.
• Collaborates with an interdisciplinary team and participates in service planning and goal reviews.
• Completes documentation of daily services, including progress notes and statistics.
• Participates in agency training and attends weekly supervision with a clinical supervisor.

Requirements

• High School Diploma required; Associate's or Bachelor's degree in an allied health field preferred.
• Minimum two years of social services experience required.
• Ability to work with clients and families in a respectful manner considering cultural differences.
• Demonstrated clinical competence and ability to react appropriately in emergencies.
• Basic computer skills and ability to complete reports accurately and timely.
• Effective communication skills with staff, clients, and the public.

Nice-to-haves

• Bilingual in Spanish preferred.
• Experience in supervising is a plus.
