About the position
The Case Manager at Providence Swedish Rehabilitation Hospital in Lynnwood, WA, plays a crucial role in the interdisciplinary team, providing care for patients in a newly opened 40-bed inpatient acute rehabilitation facility. This position focuses on coordinating patient care, developing treatment plans, and ensuring effective communication among team members and caregivers to facilitate optimal patient recovery and discharge planning.
Responsibilities
- Completes departmental orientation, initial and annual competencies.
- Consults with other departmental staff to collaborate in patient care delivery, identify barriers to care, and develop solutions.
- Completes documentation per workflow timeline and content requirements, including the Individual Plan of Care (IPoC) per CMS guidelines.
- Schedules family conferences and communicates with caregivers to keep them informed of patient progress and rehabilitation plans.
- Coordinates weekly patient care team conferences to develop and monitor treatment plans.
- Reviews the patient's assigned CMG and identifies any potential missed comorbid conditions, communicating findings to the HIM team.
Requirements
- Master's or Bachelor's degree in Social Work or as required by state.
- Certification in Case Management or Rehabilitation Nursing preferred (e.g., CCM, ARN, ACM, RN-BC).
- Minimum of 2 years of social work or case management experience in an inpatient setting, preferably in acute/rehabilitation hospitals.
- Effective oral and written communication skills in English; additional languages preferred.
- Basic computer skills in Excel, Word, Outlook, PowerPoint, etc.
