About the position

The Case Manager/Care Coordinator for the Offender Reentry Program at EAC Network is responsible for assisting clients with co-occurring substance use and mental disorders as they transition from state prison back to New York City. This full-time position involves outreach, assessment, and the development of integrated reentry service plans to ensure clients receive the necessary support and resources for successful reintegration into society.

Responsibilities

  • Administer assessment tools at DOCCS facilities, via video conference and in the community.
  • Conduct a benefits eligibility screening and assist clients in obtaining benefits such as public assistance, SSD/SSI, housing, and medical needs.
  • Develop integrated reentry service plans for clients.
  • Arrange for and implement placement into appropriate services.
  • Provide Intensive Case Management (ICM) including skills building, boundary spanning, counseling, advocacy, interactive journaling, CBT, and family-centered interventions.
  • Maintain complete client records and ensure they are available for inspection by supervision.
  • Escort clients to appointments as needed.
  • Travel to parole offices and correctional facilities to meet clients and ensure treatment compliance.
  • Maintain a caseload through ongoing client contact.
  • Attend meetings and clinical trainings.

Requirements

  • Bachelor's Degree in a relevant field of study.
  • At least one year of experience working with individuals involved with the criminal justice system, preferably those with co-occurring substance use and mental disorders.
  • Knowledge of available treatment services and ability to work cooperatively with housing, treatment, and mental health systems.
  • Good communication skills, both oral and written.
