About the position

The Case Manager - Residential Services at The Salvation Army is responsible for providing comprehensive, long-term case management services to clients in a structured life management program. This role involves assessing client needs, developing individualized program plans, and advocating for clients to access necessary services. The Case Manager will work closely with clients to monitor their progress and ensure compliance with funding requirements, while also maintaining accurate records and documentation.

Responsibilities

  • Interview applicants to determine eligibility for program enrollment based on established guidelines.
  • Conduct needs assessments and obtain pertinent information from clients.
  • Establish comprehensive long-term program goals for eligible clients based on their unique circumstances.
  • Counsel clients on modifying negative behaviors and provide encouragement.
  • Outsource clients to additional counseling resources as needed.
  • Assist clients in obtaining and maintaining self-sustaining sources of income and benefits.
  • Meet regularly with clients to discuss and evaluate their progress and personal growth.
  • Prepare and maintain accurate case records and logs for all assigned clients.
  • Facilitate individual and group discussions to promote client motivation and self-awareness.
  • Complete and articulate discharge plans for clients exiting the program.
  • Prepare statistical records and reports on services provided.

Requirements

  • High school diploma or GED.
  • Two years of experience in a social or public service environment.
  • Strong writing and communication skills in English.
  • Valid State Driver License.
  • Knowledge of social service case management principles and practices.
  • Ability to develop program plans based on client needs.
  • Ability to evaluate client progress toward program goals.
