About the position

The Case Manager at Episcopal Community Services (ECS) is responsible for providing care coordination and referral/linkage services to adults with substance use disorders. This role involves working closely with clients to support their recovery journey, linking them to relevant resources, and participating in treatment team meetings to develop and maintain treatment plans. The Case Manager plays a crucial role in improving the lives of individuals facing homelessness, mental health challenges, and substance use disorders in San Diego.

Responsibilities

  • Work with clients in need of significant care coordination and referral services.
  • Link clients with relevant resources to support their recovery.
  • Provide information, support, and follow-up services as needed.
  • Participate in treatment team meetings and contribute to treatment plans and diagnoses.
  • Develop and maintain a comprehensive database of resources for clients.
  • Complete client intakes as necessary.
  • Facilitate substance use disorder (SUD) groups as needed.
  • Ensure client records are maintained according to quality improvement guidelines.
  • Consult with community, social, health, and welfare agencies.
  • Administer internal and external rules and program standards to support client progress.
  • Perform drug testing and urinalysis specimen collections as required.

Requirements

  • Must be a Certified or Registered SUD Counselor with the state of California.
  • Bachelor's Degree in counseling, psychology, or social work from an accredited university preferred.
  • At least 2 years' experience in a clinical setting, preferably with clients with co-occurring substance use disorders.
  • Knowledge of ethics and standards relevant to client/staff interactions in therapeutic and case management environments.
  • Bilingual in Spanish preferred.

Nice-to-haves
