About the position

The Case Manager for the Child & Adolescent Program plays a crucial role in guiding clients through the establishment of benefits and accessing various resources. This position involves assessing client needs, making referrals, and ensuring that clients and their families receive the necessary support and services. The Case Manager collaborates with a multidisciplinary treatment team to monitor client behavior and functioning, while also maintaining effective communication with community agencies to foster linkages and support for clients.

Responsibilities

  • Guide clients in establishing benefits such as Medi-Cal and ACCESS.
  • Assist clients in accessing medical, dental, financial, housing, and legal resources.
  • Collaboratively assess client needs and make referrals to appropriate services.
  • Monitor clients' behavior and functioning, providing support to develop necessary skills.
  • Establish and maintain effective liaisons with community agencies.
  • Complete required documentation in a timely manner, including progress notes.
  • Participate in CFT and/or School Meetings to provide case management services.
  • Provide rehabilitation services to improve clients' functional skills and daily living skills.
  • Respond to emergency situations with good judgment and objectivity.
  • Attend training and participate in weekly individual supervision.

Requirements

  • Bachelor's degree in a mental health-related field or equivalent experience with S.E.D. children.
  • 2+ years of experience working with seriously emotionally disturbed youth and their families.
  • Bilingual in English and Spanish, with strong communication skills in both languages.
  • Experience with electronic health record systems and basic computer skills.
  • Knowledge of government programs such as SSI and Medi-Cal.
  • Ability to work effectively with a multidisciplinary treatment team.

  • Strong writing ability demonstrated in previous employment.
