About the position

New Horizons of the Treasure Coast Inc. is seeking a dedicated and compassionate Case Manager to join our Children's Community Action Team (CAT). As the largest mental health and addiction recovery provider in the Treasure Coast region, we serve over 15,000 children and adults annually through a variety of services, including inpatient crisis care, a 24-hour help line, and outpatient programs. Our mission is to improve the health and quality of life for individuals in our community, and we are looking for someone who shares this commitment to making a difference. In this role, you will be responsible for providing crisis intervention services and respite care for clients and their families. You will work closely with other team members to develop individualized service plans based on comprehensive assessments, ensuring that each plan outlines long-term desired outcomes and strategies for achieving them. As an advocate for your clients, you will link them to appropriate services and monitor their progress, serving as a vital support person throughout their journey. Additionally, you will act as a liaison between our agency and various community organizations, including the Department of Children and Families (DCF) and the Department of Juvenile Justice (DJJ). This will involve coordinating referrals and organizing community presentations to raise awareness about the services we provide. You may also assist in transporting clients to medical appointments, court hearings, and other activities as outlined in their Plan of Care. Responsibilities

  • Assist other team members with Crisis Intervention Services for the client and/or family.
  • Provide respite care for up to 4 hours (no overnight) and participate in a rotating on-call schedule.
  • Complete an individualized service plan for each client based on information gathered from assessments, including long-term desired outcomes and comprehensive strategies for assistance.
  • Function as an advocate and support person for individuals assigned to the CAT Team, linking them to appropriate services and monitoring their progress.
  • Serve as an agency liaison with DCF, DJJ, and other community agencies for coordinating referrals and organizing community presentations.
  • Assist with transporting youth or families to medical appointments, court hearings, or other related activities outlined in the Plan of Care.

Requirements

  • Bachelor's Degree in a social behavior science or related field.
  • Three (3) years of applicable experience.
  • Valid Florida Driver's License with 6 points or less in the last 5 years.
