About the position

The Case Manager for the Children's Health and Social Safety Net Initiative (CHASSI) at Catholic Charities of Orange, Sullivan & Ulster is responsible for providing direct case management services to needy and disenfranchised individuals in Orange and Sullivan County. This part-time role focuses on advocacy, information provision, referral, and short-term case management, working collaboratively with various departments to connect clients with essential services.

Responsibilities

  • Conduct initial intake utilizing Apricot platform database system.
  • Actively participate in system-wide data sharing through integrated digital referral systems (Unite Us) or other means.
  • Utilize basic Substance Use Disorder Assessments and brief bio-psychosocial intake to ensure all needs are identified comprehensively.
  • Provide education to clients and families regarding adverse childhood effects and collaborate with SUD clinicians for comprehensive services.
  • Prioritize intakes and ensure warm handoffs throughout the CCOSU system and community partners.
  • Attend Parent-Child Program training and develop effective service delivery.
  • Be part of an interdisciplinary treatment team to maximize CCOSU resources for participants.
  • Maintain current and accurate documentation of services provided to clients.
  • Answer telephones and provide necessary information.
  • Develop and maintain working relationships with providers in Orange and Sullivan Counties.
  • Attend community and service provider meetings.
  • Maintain an updated Community Resource list for client referrals.
  • Participate in outreach activities on weekends and after-hours.
  • Provide community outreach to ensure access to the program and educate about health determinants.
  • Participate in regular supervisory sessions with the immediate supervisor.
  • Attend Agency's in-service training, case conferences, and formalized coaching from the Parent-Child Program.
  • Travel to meet clients at other CCOSU offices.
  • Perform other duties as assigned by the supervisor.

Requirements

  • High School Diploma or Equivalency required with at least two years' experience working in case management with families.
  • Must possess a clean NYS Driver's License and maintain it throughout employment.

Nice-to-haves

  • Associate's degree preferred.
  • Knowledge of Social Services available in Orange and Sullivan counties.
  • Bilingual in English and Spanish.
