About the position

The Case Manager - Children's (Wraparound Facilitator) at Hegira Health, Inc. is responsible for facilitating the wraparound process for children with serious emotional disorders and their families. This role involves working collaboratively with families to identify strengths and needs, developing individualized service plans, and coordinating with community resources. The position requires flexible hours to accommodate family meetings, court appearances, and school meetings, making it a dynamic and rewarding role where no two days are the same.

Responsibilities

  • Facilitate wraparound process in accordance with Michigan Wraparound Best Practice Values.
  • Maintain current knowledge of the Wraparound Process model.
  • Complete strengths-based discovery assessments for all clients/families within 30 days of initial contact.
  • Acquire and create visual aids for assessing child and family strengths and needs.
  • Prioritize child and family needs based on life domains.
  • Identify cultural issues regarding values, preferences, beliefs, and identity of child and family.
  • Evaluate individual strengths for participation on child and family teams.
  • Complete individualized Wraparound Service Plans within 45 days of initial family meeting.
  • Develop services to facilitate Wraparound Plans and coordinate with referring agency staff and Community Team.
  • Customize strategies, supports, and services to meet unique needs of each child and family using a Person-Centered Planning approach.
  • Monitor progress of Wraparound Plans and revise as necessary based on measurable indicators of success.
  • Build on and enhance capabilities of the child and family, their community, and team members using a Strength Based Perspective.
  • Identify changing strengths and needs of child and family consistently with the national wraparound model.
  • Coordinate individual services as needed.
  • Assist Supervisor in Community Team development as assigned.
  • Report to the Community Team monthly to present wraparound family barriers for feedback.
  • Organize, schedule, and facilitate initial and weekly Child and Family Team meetings.
  • Develop and utilize community resources to meet specified needs as determined by the Child and Family Team.
  • Act as a liaison to other community agencies.
  • Complete case record documentation and follow directives from staff meetings.
  • Utilize available resources in crisis situations.

Requirements

  • Bachelor's Degree from an accredited institution.
  • Two years of experience in a human services field, with at least one year providing services to children and families.
  • Knowledge of wraparound process philosophy.
  • Working knowledge of community resources.

Nice-to-haves

  • Experience in a behavioral health setting treating children (preferred).
