About the position

The Community Youth Center of San Francisco (CYC) is dedicated to providing essential services to the youth of San Francisco, fostering a sense of belonging and equipping them with the necessary tools and experiences to thrive. The Case Manager position at the Community Assessment Referral Center (CARC) is a vital role that focuses on serving high-risk Asian and Pacific Islander youth and their families. This position is under the supervision of the Program Director and involves working directly with youth in various settings, including schools, community centers, and the Juvenile Justice Center. The CARC's mission is to serve as a single point of entry for assessment, service integration, referral, crisis intervention, and case management for arrested youth aged 11-17 in San Francisco. As a Case Manager, you will conduct comprehensive assessments to determine the needs of youth on probation and their families, ensuring that they receive the necessary support and resources. You will attend court hearings to advocate for these youth and their families, providing individual and family care management, as well as referral services. Developing individualized care plans and monitoring the implementation of treatment goals will be key responsibilities, along with ensuring service linkage to mental health services, including assessments and counseling sessions. The role also involves advocating for clients to reduce their contact with the juvenile justice system, facilitating meetings with relevant parties during intervention and reintegration processes, and maintaining official contacts with schools, community organizations, and various departments. Networking with other service providers to establish liaisons and resources is essential, as is attending client review meetings, field visits, clinical meetings, and agency meetings as required. You will also prepare program documentation and reports, support community events, and participate in activities that promote violence prevention and reduction in the community. This position may require working some evenings and weekends, as well as participating in outdoor activities.

Responsibilities

  • Conduct assessments to determine the needs of the youth on probation and their families.
  • Attend court hearings and provide advocacy in courts for youth and their families.
  • Provide individual and family care management and support and referral services.
  • Develop individualized care plans and monitor the implementation of treatment goals.
  • Ensure service linkage to mental health services including assessment and counseling sessions.
  • Advocate for clients to reduce recidivism contact with the juvenile justice system.
  • Facilitate meetings with related parties during the intervention/reintegration processes.
  • Maintain official contacts to schools, community organizations, Department of Public Health, Juvenile Probation Department, Public Defenders, and attorney offices.
  • Network with other service providers to establish liaisons and resources.
  • Attend client review meetings, field visits, clinical meetings, and agency meetings as required.
  • Work in conjunction with other CYC programs to provide effective, comprehensive services.
  • Prepare program documentation and reports.
  • Support community events, activities, and other projects that promote violence prevention and reduction in the community.
  • Other duties as assigned by management.

Requirements

  • B.A. preferred or 2+ years of experience in social services, care management, or related areas.
  • Experience in conducting interviews and assessments with youth and parents.
  • Experience working with San Francisco's diverse communities and neighborhoods.
  • Experience in conducting support groups.
  • Knowledge of the San Francisco Juvenile Justice System.
  • Knowledge of community-based agencies serving youth in San Francisco.
  • Strong interpersonal skills to work with youth, parents, and probation officers.
  • Excellent verbal and written communication skills.
  • Strong organizational and collaboration skills.
  • Ability to work some evenings, weekends, and outdoor activities.
  • Able to lift or move at least 25lbs.
  • Bilingual in English and Chinese (Cantonese and/or Mandarin) highly preferred.
  • Commitment to taking regular Covid-19 health tests as required by program health and safety guidelines and/or current health directives.
