About the position

The Case Manager position at Community Empowerment Resources (CER) involves developing clinically based treatment relationships with clients diagnosed with serious and persistent mental illness (SPMI). The role focuses on assisting clients in integrating into the community through structured recovery plans that encompass various services such as psychiatric, medical, housing, and legal coordination. The position requires flexibility in structuring workdays while maintaining a manageable caseload of 8 to 12 clients, ensuring high-quality care and support.

Responsibilities

• Develop clinically based treatment relationships with clients diagnosed with SPMI.
• Assist clients in integrating into the community using structured recovery plans.
• Coordinate psychiatric, medical, housing, and legal services for clients.
• Drive to various locations to meet clients and stakeholders in the community.
• Complete necessary documentation for client interactions.
• Transport clients when needed and when safe for transport.
• Maintain a low caseload of 8 to 12 clients to focus on high-quality care.

Requirements

• High school diploma or GED required.
• Bachelor's degree in Psychology, Social Work, Health, Nursing, Counseling, or similar field preferred.
• Under 1 year of experience in a related field.
• Valid driver's license and access to a vehicle required.
• Strong communication skills and time management abilities.

Nice-to-haves

• 6-12 months supervised experience with populations such as severely mentally ill, developmentally disabled, elderly, or children.
