About the position

The Case Manager for the Community Guardian Program at JASA is responsible for providing crisis intervention, protective services, and intensive case management to clients who are mentally and physically impaired, abused, or neglected. This role involves assessing clients' needs, developing service plans, and coordinating various services to ensure the well-being of individuals under guardianship. The position requires travel throughout New York City to meet clients and provide necessary support, while also maintaining compliance with agency policies and court requirements.

Responsibilities

  • Conduct initial assessments and monthly in-home assessments of clients' social and emotional needs.
  • Screen clients for program eligibility and conduct triage for emergency interventions.
  • Develop and implement written service plans to provide a full range of social services.
  • Arrange for direct provision of services such as homemaker, home health aide, and legal aid.
  • Disburse funds for needed goods and services through JASA relief funds.
  • Review guardianship court orders and ensure compliance with court-ordered services.
  • Advocate on behalf of clients and maintain relationships with community agencies.
  • Represent the agency at community conferences and meetings.
  • Complete required case records, reports, and statistics within mandated timeframes.

Requirements

  • Bachelor's degree in Social Work, Sociology, Psychology, or Human Services.
  • Demonstrated organizational skills and ability to manage time effectively.
  • Computer literacy, including the ability to learn and operate software.
  • Ability to travel to clients' homes using mass transit and navigate barriers in residences.
  • Understanding of the roles of race, religion, ethnicity, and individual values in serving clients.
