About the position

The Case Manager for Community Health Services at the Chinese American Planning Council (CPC) plays a vital role in supporting individuals living with HIV/AIDS within the Asian American and Pacific Islander (AAPI) communities. This position is part of a dedicated team that aims to raise awareness about HIV/AIDS, provide educational outreach, and ensure access to necessary healthcare services. The Case Manager will report directly to the Program Director and will be responsible for delivering supportive services to clients, linking them to care, and recruiting high-risk individuals for HIV/STD testing. The role requires a strong commitment to community service and the ability to navigate the complexities of healthcare access for marginalized populations. In this position, the Case Manager will conduct field visits, provide translation services, and maintain ongoing assessments of community members' situations. They will facilitate access to social services benefits and coordinate the delivery of these services to ensure that clients receive comprehensive support. The Case Manager will also be responsible for maintaining accurate records and reports as required by government funding sources, as well as developing workshops and presentations to educate the community about HIV/STD issues. Collaboration with medical and social service providers is essential, as is participation in offsite events and special projects as assigned by the Program Director. Responsibilities

  • Provide support for comprehensive service coordination to affected individuals and high-risk populations to obtain necessary information, support, and skills to access the medical system and other community resources.
  • Conduct field visits and provide translation as necessary.
  • Provide ongoing analysis and assessment of community members' situations through progress notes and assessment forms.
  • Facilitate access to and coordinate delivery of available social services benefits for community members.
  • Maintain and submit reports as required by government funding source requirements.
  • Provide support to recruit from targeted and at-risk demographics to enroll in locally developed/CDC-supported intervention.
  • Develop and maintain relationships with medical and social services providers.
  • Develop workshops/presentations to educate the community about HIV/STD issues.
  • Attend and represent the program in offsite events, work with external organizations and/or funding sources.
  • Collaborate with department staff on special projects as assigned, and other duties assigned by Program Director.

Requirements

  • Bachelor's Degree in Social Work or a human service-related field preferred.
  • Experience working in the HIV affected and/or Chinese/Asian communities is a plus.
  • Strong interest in educating the community is a plus.
  • Bilingual in Mandarin and English is a must.
  • Excellent written and oral communication skills, including interpersonal skills.
  • Ability to work in a team setting.
  • Flexibility in schedule preferred.
