About the position

The Case Manager - Community Health Worker (CHW) at Community Renewal Team is responsible for providing comprehensive care coordination to seniors, assisting them in obtaining necessary services and resources to improve their health outcomes. This role involves maintaining individual files, coordinating with healthcare providers, and advocating for clients to ensure they receive the support they need. The CHW will also engage in community outreach and education to promote health literacy and economic stability among seniors.

Responsibilities

• Provide comprehensive care coordination to customers using the STS process.
• Maintain individual records including file maintenance and purging.
• Monitor and document customers' progress and adherence to Individualized Service Plans.
• Ensure accurate entry of intake information into STS.
• Coordinate services for individuals relative to senior services programs.
• Interview customers on-site or through home visits to provide access to case management services.
• Advocate and mediate for customers with healthcare providers and state agencies.
• Assist customers with entitlement programs such as Medicare and SNAP.
• Coordinate workshops and activities for seniors to enhance health literacy and economic stability.
• Serve as a liaison to community organizations and healthcare providers.
• Refer clients for core medical services and monitor/document linkages.

Requirements

• High School Diploma or GED
• 2 years of experience working with seniors
• Bilingual in English and Spanish preferred
• Proficiency in Microsoft Office Suite
• Community Health Worker Certification or willingness to obtain within six months
• CHOICES certification preferred or willingness to obtain within six months
