About the position

The Community Resource Advocate II/Case Manager position at TLC, The Treatment and Learning Centers, is a vital role focused on supporting adults with special needs. This position is designed for individuals who are passionate about making a difference in the lives of those with disabilities. The advocate will be responsible for providing job development, job placement, and job coaching to consumers enrolled in the Supported Employment and Day Hab Programming. This role requires a commitment to fostering independence and self-advocacy among consumers, ensuring they have the necessary support to thrive in their employment endeavors. In addition to job coaching for those in the Supported Employment program, the advocate may also provide services to individuals enrolled in DORS-funded programs as directed by department leadership. The role extends to contributing support services to consumers in various programs, including Individual Support Services, Community Development Services, and Personal Supports. This multifaceted position requires a collaborative approach, working closely with team members to ensure that all consumers receive comprehensive and effective support tailored to their unique needs. Candidates should be prepared to engage with consumers in a meaningful way, demonstrating empathy and understanding of the challenges faced by individuals with disabilities. The advocate will also be responsible for maintaining accurate and thorough documentation of services provided, ensuring compliance with relevant laws and regulations regarding disability and privacy. Responsibilities

  • Provide job development, job placement, and job coaching to consumers enrolled in Supported Employment/Day Hab Programming.
  • Deliver Job Coaching services to individuals in DORS-funded programs as assigned by Department Leadership.
  • Contribute support services to assigned consumers in the Individual Support Services program.
  • Assist in the Community Development Services and Personal Supports program.

Requirements

  • A minimum of a bachelor's degree in Vocational Rehabilitation, Psychology, Special Education, or a closely related field.
  • Preferred experience working with individuals with brain injury, neurological impairments, and developmental disabilities; direct work experience may substitute for educational requirements.
  • Must have a car and maintain a valid driver's license with an acceptable driving record.
  • Must be computer literate.
  • Ability to demonstrate and apply knowledge about disabilities, including disability and privacy laws.
  • Ability to communicate effectively with consumers and staff, exhibiting effective customer service skills.
  • Ability to promote consumer self-advocacy skills and counseling skills.
  • Ability to provide well-written and comprehensive documentation.
  • Must be highly organized and able to demonstrate effective time management skills.
  • Must be able to work with a team and support team members.
