About the position

The Case Manager position at North Florida Rehabilitation Hospital involves coordinating patient care for individuals undergoing rehabilitation for various conditions. The role focuses on developing and monitoring treatment plans, facilitating communication among care teams, and ensuring superior patient outcomes in a supportive environment dedicated to community service.

Responsibilities

  • Completes departmental orientation, initial and annual competencies.
  • Consults with other departmental staff to collaborate in patient care delivery, identify barriers to care, and develop solutions.
  • Completes documentation per workflow timeline and content requirements, including the Individual Plan of Care (IPoC) per CMS guidelines.
  • Schedules family conferences and communicates with caregivers to keep them informed of patient progress and discharge plans.
  • Coordinates weekly patient care team conferences to develop and refine treatment plans.
  • Reviews the patient's assigned CMG and identifies any potential missed comorbid conditions, communicating findings to the HIM team.

Requirements

  • Current Registered Nurse or Social Work licensure or Healthcare professional licensure as Respiratory Therapist, Physical Therapist, Speech Language Pathologist, or Occupational Therapist.
  • Certification in Case Management or Rehabilitation Nursing preferred (e.g., CCM, ARN, ACM, RN-BC).
  • Minimum of 2 years social work or case management experience in an inpatient setting, preferably in acute/rehabilitation hospitals.
  • Effective oral and written communication skills in English; additional languages preferred.
  • Basic computer skills in Excel, Word, Outlook, PowerPoint, etc.
