About the position

The Case Manager is a vital member of the patient-centered care team at Henry Ford Jackson Hospital, responsible for the identification, assessment, planning, facilitation, care coordination, evaluation, and advocacy for patients' healthcare needs. The role aims to promote patient safety, quality of care, and cost-effective outcomes, particularly through effective discharge planning services that ensure a safe transition to the next level of care for patients who have experienced critical events or diagnoses.

Responsibilities

• Collaborate with the care team to identify and assess patients' healthcare needs.
• Plan and facilitate care coordination and evaluation of services.
• Advocate for options and services to meet individual and family health care needs.
• Provide effective discharge planning services to support safe transitions in care.
• Address the needs of patients requiring management strategies to optimize health outcomes.
• Interact with internal and external customers at all levels of the organization.

Requirements

• Bachelor's degree in nursing or a Master's degree in Social Work.
• Minimum of three years of clinical experience.
• Excellent verbal communication and written documentation skills.
• Strong customer service and interpersonal skills.
• Strong problem-solving, analytical, and decision-making skills.
• Proficient computer skills and knowledge.
• Experience in discharge planning, home health care, rehabilitative medicine, or managed care preferred.
• Knowledge of preventive service guidelines and clinical practice guidelines.
• Understanding of social determinants of health and their impact on patient wellbeing.

Nice-to-haves

• Certification in Case Management (CCM) or Accredited Case Manager (ACM) preferred.
