About the position

The Case Manager at Total Care Services, Inc. plays a vital role in the Coordination of Community Services program, focusing on supporting individuals with intellectual and developmental disabilities and their families in Maryland. This position involves working collaboratively with clients to identify their needs and goals, providing information about community resources, and ensuring access to necessary services. The role is hybrid, allowing for both remote work and local community visits, and requires strong advocacy and case management skills.

Responsibilities

  • Work collaboratively with clients and their families to identify needs, goals, and preferences.
  • Provide information about community resources and support services.
  • Empower clients to make informed choices and plan for their future.
  • Assist clients in accessing service delivery systems, resources, and supports.
  • Monitor and evaluate services to ensure they meet clients' needs and preferences.

Requirements

  • Bachelor's degree in human services field (psychology, social work, sociology, nursing, counseling, criminal justice, etc.).
  • Minimum of 1 to 3 years of Clinical Case Management experience.
  • Proven experience in Case Management.
  • Strong advocacy for individuals with intellectual and developmental disabilities.
  • Excellent written and verbal communication skills.
  • Excellent organization and time management skills.
  • Technologically savvy.
  • Must be a licensed driver with reliable transportation, valid insurance, and a safe driving record.
  • Must be fully vaccinated and pass all pre-employment qualifications.
