About the position

The position involves responsibilities related to organizing, updating, and filing clinical documentation. The individual will gather and complete all clerical and generic portions of materials necessary for the admission, recertification, and discharge of patients. Additionally, the role includes preparing, reviewing, and disassembling patient records after the patient has been discharged. The candidate will also be responsible for answering phone calls when the Supervisor and/or Case Manager is out of the office or otherwise unavailable, and making patient home visits as needed.

Responsibilities

  • Organizing, updating, and filing clinical documentation.
  • Gathering and completing all clerical and generic portions of materials necessary for admission, recertification, and discharge of patients.
  • Preparing, reviewing, and disassembling patient records after discharge.
  • Answering phone calls when the Supervisor and/or Case Manager is unavailable.
  • Making patient home visits as needed.

Requirements

  • Graduate of an accredited practical nurse or vocational nursing program.
  • Currently licensed as a licensed practical nurse in the State.
  • Prefer at least 1-year nursing experience, preferably in community/home health or medical/surgical.
  • Complies with accepted professional standards and practice.
  • Possesses and maintains good physical stamina and mental health.
  • Self-directed with the ability to work with little supervision.
  • Good organizational skills.
  • Flexible and cooperative in fulfilling role obligations.
  • Must be a licensed driver with an insured automobile in good working order.
  • Excellent observation and communication skills.
  • Ability to politely answer phones in a proper and orderly fashion.
