About the position

The Case Manager for the Court to College Program at Los Angeles Room & Board is a full-time role focused on providing high-quality services to formerly incarcerated students. This position involves case management, program administration, and collaboration with various stakeholders to support students in achieving their academic and personal goals. The Case Manager will be responsible for the health and safety of students, conducting assessments, and creating court reports for legal proceedings.

Responsibilities

  • Serve as a specialist to organization staff, providing information on mental health topics and community resources for justice-involved youth.
  • Support the mental health training of organization staff.
  • Complete client interviews, intakes, service plans, and administer appropriate assessments.
  • Meet with students 1:1, at least once per week, for the purposes of building rapport and assisting with linkages to behavioral health services.
  • Identify, refer, and connect clients and families to appropriate community resources.
  • Coordinate with staff and external partners to support clients' fulfillment of court-ordered and program responsibilities.
  • Develop relationships with community agencies to foster effective coordination of services for clients.
  • Complete specified documentation in an accurate and timely manner and in compliance with established organizational protocol and productivity standards.
  • Complete mental health and biopsychosocial assessments and make recommendations for ongoing treatment and care.
  • Utilize evidence-based treatment modalities to support the needs of all clients.
  • Engage in crisis intervention, as necessary.
  • Maintain case notes to reflect weekly 1:1 case management meetings with students.
  • Attend regularly scheduled 1:1s with supervisor.
  • Assist with data assessment and evaluation according to an articulated plan.

Requirements

  • A minimum of two years of experience providing case management and therapeutic services.
  • Training in evidence-based treatment modalities.
  • Valid RN License (Preferred).
  • Certified Case Manager certification (Preferred).
  • Clinical Social Worker certification (Preferred).
