About the position

Highland Rivers Behavioral Health is seeking a dedicated Behavioral Health Case Manager to join our team. This position is crucial in providing direct care and support services to adults in crisis, particularly those impacted by mental illness, substance use disorders, and intellectual developmental disabilities. The Case Manager will play a vital role in the Crisis Stabilization Unit (CSU), assisting in the development and implementation of discharge planning activities for individuals receiving treatment. This role requires a compassionate individual who is ready to make a difference in the lives of those in need. The Behavioral Health Case Manager will work closely with the treatment team to link individuals to aftercare services, ensuring that they receive the necessary support post-discharge. This includes engaging clients in aftercare services and participating in follow-up contacts to enhance their active participation. The Case Manager will also be responsible for collecting information for individual intake and admission, conducting interviews with clients, their families, and relevant community agencies to gather comprehensive data that will inform treatment plans. In addition to these responsibilities, the Case Manager will assist the treatment team in identifying individual strengths and weaknesses, which will aid in the development of tailored aftercare plans. This position requires a proactive approach to monitoring the physical and emotional wellbeing of clients, reporting any unusual behavior or physical complaints to the direct supervisor or Charge Nurse. The Case Manager will also be involved in the admission and discharge processes, ensuring that all necessary protocols are followed, including obtaining vital signs and conducting safety searches. Overall, this role is integral to the holistic care and recovery of individuals in crisis, and it offers an opportunity to contribute positively to the community.

Responsibilities

  • Assist individuals with identifying aftercare needs and developing a discharge plan through interviews with the individual and family when appropriate, the treatment team and clinical staff.
  • Participate in treatment team to assist with discharge planning needs.
  • Provide linkage to aftercare services by arranging appointments, transportation and supports.
  • Provide follow up and assistance to improve engagement in aftercare services and reduce recidivism.
  • Monitor the individual's physical and emotional wellbeing and report unusual behavior or physical complaints to direct supervisor or Charge Nurse.
  • Assist in the admission and discharge process, including obtaining vital signs, safety search of individual and his/her personal belongings as well as orienting new individuals to the unit.
  • Maintain clinical records and documentation according to agency policies.
  • Enhance professional growth and development through participation in educational programs, in-service meetings and trainings.
  • Assist in prevention and managing crisis situations.
  • Respond appropriately to crisis situations that may involve physical intervention including the use of seclusion/restraint.
  • Provide direct care for individuals as needed.
  • Transport individuals to approved designated location at discharge.
  • Other responsibilities and tasks as may be assigned.

Requirements

  • Experience in behavioral health or a related field is preferred.
  • Strong communication and interpersonal skills to effectively interact with clients and their families.
  • Ability to work collaboratively within a treatment team.
  • Knowledge of mental health and substance use disorders.
  • Skills in crisis intervention and management.

Nice-to-haves

  • Certification in case management or related field.
  • Experience working with individuals with intellectual and developmental disabilities.
