Highland Rivers Behavioral Health is seeking a dedicated Behavioral Health Case Manager to join our team. This position is crucial in providing direct care and support services to adults in crisis, particularly those impacted by mental illness, substance use disorders, and intellectual developmental disabilities. The Case Manager will play a vital role in the Crisis Stabilization Unit (CSU), assisting in the development and implementation of discharge planning activities for individuals receiving treatment. This role requires a compassionate individual who is ready to make a difference in the lives of those in need. The Behavioral Health Case Manager will work closely with the treatment team to link individuals to aftercare services, ensuring that they receive the necessary support post-discharge. This includes engaging clients in aftercare services and participating in follow-up contacts to enhance their active participation. The Case Manager will also be responsible for collecting information for individual intake and admission, conducting interviews with clients, their families, and relevant community agencies to gather comprehensive data that will inform treatment plans. In addition to these responsibilities, the Case Manager will assist the treatment team in identifying individual strengths and weaknesses, which will aid in the development of tailored aftercare plans. This position requires a proactive approach to monitoring the physical and emotional wellbeing of clients, reporting any unusual behavior or physical complaints to the direct supervisor or Charge Nurse. The Case Manager will also be involved in the admission and discharge processes, ensuring that all necessary protocols are followed, including obtaining vital signs and conducting safety searches.
