About the position

The Case Manager (Health Coach) at FreedomCare plays a crucial role in delivering home care services to patients in Connecticut. This hybrid position involves both remote work and periodic in-person assessments, ensuring that patients receive the necessary care while promoting their independence. The Case Manager will work closely with caregivers and waiver members to provide a seamless customer service experience, monitor care quality, and facilitate communication among the care team.

Responsibilities

  • Schedule and perform initial home assessments (virtual and in-person)
  • Serve as a field expert to support waiver member and caregiver onboarding experience
  • Perform required 'home inspections' to ensure safety and compliance of participants' home and surroundings
  • Partner with Care team to monitor Daily Notes submitted by the caregiver
  • Maintain contact with the caregiver, virtual or in person, based on the needs identified during the initial assessment
  • Visit the waiver member and caregiver with the waiver Case Manager on a semi-annual basis or as required
  • Communicate any concerns regarding the waiver member or caregivers' health, surroundings, safety, or unmet needs to staff RN
  • Track all activities through entries into CRM (Salesforce)
  • Perform other duties as required or assigned

Requirements

  • A bachelor's degree or equivalent work experience
  • Two + years' experience working with frail or elderly disabled adults and caregivers
  • 2-3 years Inside Sales, call center, telephonic outreach, customer service or other similar experience
  • 1-3 years' medical field experience and/or case management, home health
  • Strong verbal and written communication skills
  • Excellent time management, critical thinking and active listening skills
  • Strong proficiency in computer software navigation; Salesforce experience is a plus
  • Ability to exercise judgement independently and voice noticeable issues or trends
  • Possesses a high level of empathy and emotional intelligence
