About the position

The position at Virginia Mason Franciscan Health Rehabilitation Hospital involves being a vital member of an interdisciplinary team dedicated to providing superior patient care in a state-of-the-art rehabilitation facility. The role focuses on coordinating patient care, collaborating with staff, and ensuring effective communication with patients and their families throughout the rehabilitation process.

Responsibilities

  • Completes departmental orientation, initial and annual competencies.
  • Consults with other departmental staff to collaborate on patient care delivery and develop solutions to barriers to care or discharge.
  • Completes documentation per workflow timeline and content requirements, including the Individual Plan of Care (IPoC) per CMS guidelines.
  • Schedules family conferences and communicates with caregivers to keep them informed of patient progress and rehabilitation plans.
  • Coordinates weekly patient care team conferences to develop and monitor treatment plans to achieve patient goals and outcomes.
  • Reviews the patient's assigned CMG and identifies any potential missed comorbid conditions, communicating findings to the HIM team.
  • Performs other duties as assigned.

Requirements

  • Masters or Bachelors in Social Work or as required by state.
  • Certification in Case Management or Rehabilitation Nursing preferred (e.g., CCM, ARN, ACM, RN-BC).
  • Minimum of 2 years social work or case management experience in an inpatient setting highly preferred; acute/rehabilitation hospital experience preferred.
  • Effective oral and written communication skills in English; additional languages preferred.
  • Basic computer skills in Excel, Word, Outlook, PowerPoint, etc. required.

Nice-to-haves

  • Certification in Case Management or Rehabilitation Nursing (e.g., CCM, ARN, ACM, RN-BC).
  • Experience in acute/rehabilitation hospital settings.
