About the position

We are seeking an organized and compassionate Case Manager to join our team and provide support and guidance to clients in need. The Case Manager will be responsible for working with clients to identify their needs and goals, developing and implementing action plans, and providing ongoing support and monitoring to ensure progress. The successful candidate will have strong communication and problem-solving skills and be able to work collaboratively with other team members. This role is crucial in helping clients navigate their challenges and achieve their desired outcomes, making a positive impact on their lives. In this position, you will work closely with clients to conduct assessments and gather relevant information that will inform the development of personalized action plans. You will coordinate with other service providers as necessary to ensure that clients receive comprehensive support. Ongoing monitoring of client progress is essential, which includes conducting follow-up meetings and check-ins to address any emerging needs or concerns. Maintaining accurate and up-to-date records of client interactions and progress is also a key responsibility, ensuring that all documentation is thorough and compliant with organizational standards. Collaboration with other team members is vital to ensure continuity of care and services. You will be expected to participate in team meetings and contribute to discussions about client care strategies. Additionally, you may be assigned other duties as needed to support the overall mission of the organization and enhance client outcomes.

Responsibilities

Requirements

  • Bachelor's degree in social work, psychology, or a related field
  • Minimum of 2 years of experience as a case manager or in a similar role
  • Strong communication and problem-solving skills
  • Ability to multitask and handle a high-volume workload
  • Ability to work collaboratively with other team members
  • Valid driver's license and reliable transportation are a plus
