About the position

This position works with adults who have been diagnosed with a severe mental illness and/or co-occurring disorder. The role involves providing a variety of services for identified needs including but not limited to: income (employment, SSI/SSDI benefits), insurance, housing, vocational or educational needs, access to reliable transportation, food and clothing resources, activities of daily living including personal grooming and hygiene, substance use education and appropriate referrals, informal supports and leisure activities, crisis assessment and safety planning, and referrals to legal services. Additionally, the position requires educating individuals or groups on a wide variety of skills related to topics such as money management, symptom education and reduction, interpersonal relationships, maintaining a household, and smoking cessation. Crisis intervention and management may also be necessary. Services range in intensity from monthly contact to weekly contact and can be provided in-person or via telehealth platforms. The role requires gathering and completing all CCBHC data metrics as needed, along with fulfilling all duties and responsibilities outlined in the primary CM core job description.

Responsibilities

  • Provide a variety of services for identified needs including income, insurance, housing, vocational or educational needs, access to reliable transportation, food and clothing resources, activities of daily living, substance use education, crisis assessment, and referrals to legal services.
  • Educate individuals or groups on skills related to money management, symptom education, interpersonal relationships, maintaining a household, and smoking cessation.
  • Provide crisis intervention and management as needed.
  • Meet clients where they are in terms of current mental health symptoms and treatment preferences.
  • Gather and complete all CCBHC data metrics as needed.

Requirements

  • Case Manager II: Must be a high school graduate or have a HS equivalent + 36 months experience in behavioral health field OR 60 hours college credit in any field + 12 months of behavioral health experience required.
  • Case Manager III: Bachelor's Degree in any field + 6 months experience in a behavioral health field required.
  • Case Manager IV: Bachelor's degree in social science field required Case Management II certification or obtain by taking the first available Case Management Certification course provided by ODMHSAS after hire.
  • Must have a strong commitment to the right and ability of each person served to live in normal community residences.
  • Skills and competence to establish supportive trusting relationships with persons with mental illnesses and/or substance abuse issues.
  • Must possess a valid Oklahoma Driver License and satisfactory driving record.

Nice-to-haves

  • Previous background working in community mental health, inpatient psychiatric facilities, criminal justice, home health, or similar settings.
  • Strong candidates with a passion for helping individuals with severe mental illness.
