About the position

NFI Massachusetts Inc. is seeking a Case Manager/Family Support Worker for its Foundations for Families program. This role involves providing home-based treatment, community outreach, comprehensive assessment, counseling, case coordination, and crisis intervention services for youths and families with significant social, emotional, behavioral health, medical, or trauma-related needs. The position requires lived experience as a caregiver and aims to support families in navigating child and family serving systems while maximizing their strengths and avoiding out-of-home placements.

Responsibilities

  • Conduct intakes for new referrals to the program.
  • Educate and support caregivers in navigating systems of care and accessing appropriate care for their child/grandchild/etc.
  • Assist and support caregivers in problem-solving and advocacy skills.
  • Assist in locating and accessing resources.
  • Support caregivers in developing strategies for self-care and identifying respite care providers.
  • Provide in-the-moment modeling and coaching to increase parenting skills and sense of competency.
  • Create a strength-based client-focused assessment and treatment planning with guidance from youth, family, caregivers, and other supports.
  • Provide individual and family therapeutic supports as defined by the client's service plan.
  • Work collaboratively with the clinician and/or other members of the treatment team.
  • Facilitate and/or participate in case conferences and treatment meetings as appropriate.
  • Write necessary progress notes, aftercare plans, service plans, and reviews as defined by the program contract.
  • Transport clients as needed to court, appointments, and home visits.
  • Assist other staff in crisis prevention and crisis management.
  • Perform all duties in accordance with the agency's policies and procedures.

Requirements

  • Must have lived experience as a parent/caregiver of a youth with significant social, emotional, behavioral health, medical, or trauma-related needs.
  • Bachelor's degree preferred.
  • Flexible schedule including evening and weekend availability.
  • Excellent interpersonal, organizational, and writing skills required.
  • Valid driver's license as travel is required.
