About the position

The Case Manager for Adults with Intellectual Disabilities at Trinity Services, Inc. is responsible for the development, implementation, and coordination of Individual Service Plans for individuals with developmental disabilities. This role involves maintaining program data, advocating for individuals' rights, and ensuring compliance with regulatory standards. The Case Manager will work closely with a team to provide support and guidance, facilitating community integration and empowerment for the individuals served.

Responsibilities

  • Convene and chair the Community Support Team Meetings to develop and update Individual Service Plans.
  • Guide and lead the Community Support Team in treatment and habilitation methods.
  • Identify and respond to gaps in service provision.
  • Ensure that services are being correctly implemented and continue to meet the needs of the individuals served.
  • Provide personal support and assistance in gaining access to vocational services, recreation and leisure activities, and necessary consultant services.
  • Advocate for the individual's rights and services.
  • Understand and apply related regulatory standards including Illinois Department of Human Services Community Integrated Living Arrangements rules and regulations.
  • Provide pre-service and in-service training to Independent Living Counselors and other staff as indicated.
  • Review individual service plans on a monthly basis for each individual assigned; complete necessary forms and reports.
  • Conduct programmatic meetings as assigned and indicated.
  • Make suggestions for program or Agency improvements and share ideas with colleagues.
  • Seek opportunities for personal development and apply relevant clinical information.

Requirements

  • Bachelor's Degree in a Human Services field.
  • At least one year of experience working with individuals with developmental disabilities.
  • Demonstrated ability to supervise or coordinate professional, non-degreed, and support staff in developing and implementing Individual Service Plans.
  • Ability to communicate clearly, both verbally and in writing.
