About the position

The Foster Care Case Manager at Necco plays a crucial role in supporting children in foster care and their foster families. This position involves working closely with children and foster parents to help them achieve their goals and ensure their growth and development. The Case Manager will engage in various settings, including offices, community locations, schools, and foster homes, to provide necessary resources and support for permanency.

Responsibilities

  • Participate in the referral matching process for children and families, advocating for the best possible home match.
  • Facilitate and develop each child's Initial Treatment Plan and Comprehensive Treatment Plan.
  • Ensure completion of appropriate assessments, such as Biopsychosocial and Independent Living assessments.
  • Refer foster children to applicable support services, including behavioral health and independent living.
  • Obtain initial legal documentation prior to placement or service delivery.
  • Develop appropriate safety plans during intake.
  • Assist in the implementation, tracking, and monitoring of the Comprehensive Treatment Plan for foster children.
  • Establish relationships with foster parents and children through training and support.
  • Ensure compliance with state and licensure regulations regarding contact with foster children.
  • Serve as a liaison between Necco and placement agencies, as well as community resource entities.
  • Provide transportation as needed for foster children.
  • Participate in service meetings with community support representatives, guardians, state workers, and other invested parties.
  • Establish relationships with school personnel to ensure children meet educational goals.
  • Monitor and review documentation to ensure compliance with state, licensure, and accreditation requirements.
  • Participate in a weekly rotating on-call schedule to provide advisory support to children and foster parents.
  • Document and report on-call activities and follow up with team leaders.
  • Report any instances of non-compliance or abuse to supervisors or designated authorities immediately.
  • Complete and submit Medicaid documentation as appropriate.
  • Ensure accurate data entry into the Electronic Health Record according to state rules and company protocol.
  • Support teammates to ensure all foster children receive standard services from Necco.

Requirements

  • Must be at least 21 years of age.
  • Current LSW or LPC license or a Bachelor's Degree in a human services-related field.
  • Minimum of 2 years of experience working with children with emotional or behavioral problems.
  • Valid Driver's License and Limited Liability Auto Insurance Coverage of 100/300/100.
  • Strong organizational and written/oral communication skills.
  • Ability to navigate and execute various software applications, including Microsoft Office and Electronic Health Records.
  • Successful completion of all required criminal background checks and drug screenings.

Nice-to-haves

  • Bachelor's Degree in Human Services preferred.
