About the position

The Case Manager at PAM Health is responsible for the interdisciplinary coordination of care for a designated patient population. This role involves reviewing inpatient admission records to ensure proper utilization of hospital resources and determining the appropriate level of care. The Case Manager also assesses discharge needs and coordinates discharge plans while collaborating with the interdisciplinary team to achieve desired financial and quality outcomes.

Responsibilities

• Coordinate interdisciplinary care for designated patient populations.
• Review inpatient admission records for proper resource utilization.
• Determine appropriate level of care for admissions.
• Assess discharge needs and coordinate discharge plans.
• Collaborate with the interdisciplinary team to achieve financial and quality outcomes.
• Maintain and enhance payor relationships.

Requirements

• Valid RN License preferred.
• Current BLS certification required.
• Three to five years of inpatient experience, preferably in an acute, IRF, or LTACH setting.

Nice-to-haves

• Licensure in the state where the hospital resides is preferred.
• Professional disciplines may include occupational therapist, physical therapist, registered nurse, respiratory therapist, speech therapist, or licensed social worker.
