About the position

The Case Manager Generalist at Volunteers of America Los Angeles is a vital role dedicated to providing comprehensive case management services to clients classified as low acuity. This position serves as the primary point of contact for clients, coordinating a wide array of services and care tailored to meet their individual needs. The Case Manager Generalist is responsible for managing a caseload of individuals requiring various human services resources, ensuring that each client receives the full spectrum of available support. In this role, the Case Manager Generalist conducts thorough screenings and assessments to gather essential information regarding the clients' functional, environmental, psycho-social, financial, employment, housing, educational, and health circumstances. By identifying barriers that clients face, the Case Manager develops personalized case plans aimed at addressing both short- and long-term needs. This includes coordinating individualized planning sessions with clients to ensure that their specific requirements are met effectively. Additionally, the Case Manager Generalist provides service linkages and support systems, facilitating access to necessary resources. They are responsible for organizing case conferences with residents, consulting with the service team, and monitoring the progress of clients against their case plans. Advocacy is a key component of this role, as the Case Manager represents clients before judicial, community, social service, and administrative bodies as needed. They also facilitate support groups and encourage client participation in various programs, fostering a supportive environment for those in need.

Responsibilities

  • Conduct comprehensive screenings and assessments with individuals or families to collect relevant information.
  • Assess barriers facing clients and develop tailored case plans.
  • Coordinate individualized planning with clients to address short- and long-term needs.
  • Provide service linkages and support systems to ensure identified needs are met.
  • Organize case conferences with residents and consult with the service team.

Requirements

  • High school diploma or GED required.
  • Completion of VOALA Case Management training within 6 months of hire.
  • Minimum 1-year general experience in case management or a similar social services role, or a Bachelor's Degree in a related field in lieu of experience.

Nice-to-haves

  • Bachelor's Degree in Social Work, Social Services, Public Administration, Public Health, or a related field preferred.
  • 1 year of experience working with the target population.
  • Experience with HMIS navigation.
