About the position

The Case Manager Generalist at Volunteers of America Los Angeles plays a crucial role in providing case management services to clients classified as low acuity. This position serves as the primary point of contact for coordinating services and care for assigned clients, ensuring that they receive the necessary support to improve their life situations. The Case Manager Generalist is responsible for managing a caseload of individuals who require a diverse range of human services resources, and they are tasked with delivering the full spectrum of services available to these clients. In this role, the Case Manager Generalist conducts comprehensive screenings and assessments with individuals or families to gather essential information regarding their functional, environmental, psycho-social, financial, employment, housing, educational, and health needs. By assessing the barriers that clients face, the Case Manager develops tailored case plans that address both short- and long-term needs. This involves coordinating individualized planning with clients to ensure that their identified needs are met effectively. Additionally, the Case Manager Generalist is responsible for linking clients to services and support systems, facilitating case conferences with residents, consulting with the service team, and monitoring the progress of clients on their case plans. They act as advocates for clients before various bodies, including judicial, community, social service, and administrative entities, and may facilitate support groups to encourage client participation in their own care. This position is vital in helping clients navigate the complexities of social services and achieve their personal goals.

Responsibilities

  • Conduct comprehensive screenings and assessments with individuals or families to collect functional, environmental, psycho-social, financial, employment, housing, educational, and health information.
  • Assess barriers facing the clients and develop a case plan.
  • Coordinate individualized planning with clients to meet short- and long-term needs.
  • Provide service linkages and support systems to ensure identified needs are met.
  • Coordinate case conferences with residents, consult with service team, monitor progress of clients on case plan, and assist clients to meet the agreed upon goals.
  • Act as an advocate for clients before judicial, community, social service, and administrative bodies as needed; facilitate support groups and client participation as appropriate.

Requirements

  • Must be able to pass a fingerprint clearance, background check, including criminal history, personal references, employment and education verifications.
  • High School diploma or GED required.
  • Completion of VOALA Case Management training within 6 months of hire.
  • Minimum 1-year general experience in case management or similar social services role, or a Bachelor's Degree in a Social Services related field in lieu of experience.

Nice-to-haves

  • Bachelor's Degree in Social Work, Social Services, public administration, public health or related field.
  • 1 year of experience working with target population.
  • HMIS navigation experience.
