About the position

The Case Manager Generalist at Volunteers of America Los Angeles is responsible for providing essential case management services to clients classified as low acuity. This role serves as the primary point of contact for coordinating services and care for assigned clients, ensuring that they receive the necessary support to address their diverse needs. Each Case Manager Generalist manages a caseload of individuals requiring a wide range of human services resources, delivering comprehensive services tailored to each client's situation. In this position, the Case Manager Generalist conducts thorough screenings and assessments with individuals or families to gather critical information regarding their functional, environmental, psycho-social, financial, employment, housing, educational, and health circumstances. By assessing the barriers faced by clients, the Case Manager develops personalized case plans aimed at meeting both short- and long-term needs. This includes coordinating individualized planning with clients and providing service linkages and support systems to ensure that identified needs are effectively addressed. Additionally, the Case Manager Generalist is responsible for organizing case conferences with residents, consulting with the service team, and monitoring the progress of clients on their case plans. They assist clients in achieving the goals set forth in their plans and act as advocates for clients before judicial, community, social service, and administrative bodies as necessary. The role may also involve facilitating support groups and encouraging client participation in various activities.

Responsibilities

  • Conduct comprehensive screenings and assessments with individuals or families to collect relevant information.
  • Assess barriers facing clients and develop tailored case plans.
  • Coordinate individualized planning with clients to meet their short- and long-term needs.
  • Provide service linkages and support systems to ensure identified needs are met.

Requirements

  • Must be able to pass a fingerprint clearance and background check, including criminal history, personal references, employment, and education verifications.
  • High school diploma or GED is required.
  • Completion of VOALA Case Management training within 6 months of hire is necessary.
  • Minimum of 1 year of general experience in case management or a similar social services role, or a Bachelor's Degree in a related field in lieu of experience.

Nice-to-haves

  • Bachelor's Degree in Social Work, Social Services, Public Administration, Public Health, or a related field.
  • 1 year of experience working with the target population.
  • Experience with HMIS navigation.
