Responsibilities

  • Provide direct case management for all assigned patients.
  • Act as a liaison and advocate for patients and their families.
  • Ensure timely delivery of quality services from the patient care team.
  • Collaborate with government and community agencies to support patient needs.
  • Monitor patient progress and track clinical outcomes.
  • Maintain patient confidentiality and advocate for patients' rights.

Requirements

  • Bachelor's degree in social work, Psychology, Sociology, Public Health, or a related field from an accredited college or university.
  • One year of experience in human services, behavioral health, or case management, preferably in a healthcare environment.
  • Current CPR certification from an accredited certifying body.
  • Reliable transportation for operational reassignments during work hours.
  • Valid driver's license and compliance with Arizona vehicle insurance requirements.
  • Obtain and maintain Certified HEAPlus user certification with AHCCCS upon employment.
  • Gain and maintain certification as a Certified Application Counselor for the ACA within six months of employment.

Nice-to-haves

  • Master's degree in social work, Psychology, Sociology, Public Health, or a related field from an accredited college or university.
  • Three years of experience in human services, behavioral health, or case management, preferably in a healthcare environment.
  • Bilingual in English and Spanish, with proficiency in speaking, reading, and writing.
