About the position

The Health Promotion Case Manager at Lutheran Social Services of the Southwest (LSS-SW) is responsible for developing and implementing health-focused programs aimed at improving the well-being of refugees and ORR-eligible individuals in Tucson. This role involves collaboration with various stakeholders to facilitate access to healthcare services and provide culturally appropriate health resources, ensuring that clients can navigate health-related systems effectively.

Responsibilities

  • Provide case management support, helping refugees navigate state and local systems independently.
  • Schedule and coordinate healthcare services, transportation, and interpretation for clients.
  • Advocate for culturally and linguistically appropriate healthcare services.
  • Track client outcomes, prepare reports, and gather data on service delivery and program impact.
  • Design health programs focused on preventive care, education, and healthcare access.
  • Lead health education workshops, community outreach, and events for wellness promotion.
  • Develop educational materials and refer clients to screenings, vaccination drives, and other health events.
  • Build and maintain partnerships with community agencies, healthcare providers, and government agencies.
  • Attend community meetings, health fairs, and networking events as an advocate for refugee health needs.
  • Maintain professional credentials and ensure adherence to safety protocols.

Requirements

  • High school diploma or GED required.
  • Bachelor's degree (B. A.) from a four-year college or university in social work, sociology, psychology, or a related field preferred.
  • Experience in case management.
  • Working with diverse communities, especially refugees, preferred.
  • Experience in health promotion/administration and ability to advocate for culturally appropriate healthcare.
  • Strong interpersonal and organizational skills.
  • Ability to collaborate with diverse individuals in a multicultural environment, collaborating with people from diverse backgrounds non-judgmentally.
  • Commitment to promoting client-centered service and respect for diversity in all interactions.
  • Negotiation and conflict resolution skills.
  • Ability to manage time effectively, work independently, and manage confidential information with discretion.
  • Proficient with computers and related business electronics, systems, and software, including Microsoft Office products.
  • Professional communications in all forms, strong customer service skills, effective listening, ability to ascertain needs, and exercise good judgment.
