About the position

The Case Manager for the Highbridge Overlook Supportive Housing Program at CAMBA plays a crucial role in supporting individuals with serious mental health illnesses who are experiencing homelessness. This position involves maintaining professional relationships with clients, conducting assessments, and assisting tenants in achieving their independent living goals through resource identification and advocacy. The Case Manager will work collaboratively with tenants to develop and revise living plans, monitor progress, and ensure compliance with privacy and security policies.

Responsibilities

  • Maintain professional relationships with clients and ensure client confidentiality.
  • Practice Universal Precautions/Standard Protocol & Procedures.
  • Comply with Federal, State, City, and CAMBA security and privacy policies.
  • Review documentation to establish tenants' eligibility for the program and maintain tenant files.
  • Conduct initial intake and assessment of tenants and their families' needs, along with periodic re-assessments.
  • Prepare and revise independent living plans in collaboration with tenants, including setting short-term and long-term goals.
  • Assist tenants in identifying community resources and making referrals to appropriate services.
  • Support tenants in overcoming barriers to their goals and advocate for their self-sufficiency.
  • Monitor tenants' progress through regular contact and document via progress notes.
  • Participate in case conferences and follow up with tenants and referral organizations regarding progress.
  • Provide required information for reports and may recommend case closures based on tenant progress or eligibility.
  • Act as a tenant liaison with outside organizations regarding education, healthcare, housing, and legal issues.
  • Schedule appointments for tenants with referral organizations and may escort them to appointments.
  • Assist tenants in completing applications for benefits and entitlements, processing applications on their behalf.
  • Evaluate living conditions of tenants through home visits and prepare marketing materials for the program.
  • Prescreen tenants for eligibility and schedule intake appointments, as well as input tenant data into an automated database.

Requirements

  • Bachelor's degree (e.g., B.A., B.S.W.)
  • Experience working with the Seriously and Persistently Mentally Ill (SPMI) population or equivalent experience.
