About the position

The Home Care Case Manager at LifePath is responsible for evaluating and implementing Home Care services for eligible clients. This role involves assessing client needs, developing service plans, and advocating for clients with various agencies. The Case Manager will work collaboratively with clients and their support systems to foster independence and ensure access to necessary services, all while adhering to regulatory standards.

Responsibilities

  • Assess client needs in collaboration with caregivers and agencies.
  • Develop individualized service plans with clients and their support systems.
  • Coordinate services with internal programs and authorize funded services.
  • Make referrals to community-based agencies as needed.
  • Collaborate with formal providers to ensure service plan coordination.
  • Maintain accurate documentation in the Social Assistance Management System (SAMS).
  • Assist clients and families in problem-solving and accessing services.
  • Facilitate team meetings and case conferences as necessary.
  • Stay informed about available services for elders in the service area.
  • Serve as an advocate for clients to obtain additional services.
  • Participate in staff and interdisciplinary team meetings.
  • Provide office coverage and participate in on-call emergency coverage.

Requirements

  • Bachelor's degree preferred; previous experience in health or human services required.
  • Valid driver's license and insured, reliable vehicle.
  • Good written and oral communication skills.
  • Strong time management and recordkeeping abilities.
  • Excellent computer skills for data entry and record maintenance.
  • Willingness to work with elders and make comprehensive needs assessments.
  • Flexibility to meet changing program demands.

Nice-to-haves

  • License in Social Work is a plus.
  • Knowledge of the LifePath service area is preferred.
  • Bilingual skills are encouraged and may provide a pay differential.
