About the position

The Case Manager for Homeless Services at Horizon House, Inc. is responsible for providing engagement and case management support to individuals experiencing homelessness. This role involves home-based visitations and community services, ensuring that participants receive individualized support to maintain permanent housing. The Case Manager will work closely with a multidisciplinary team and engage in outreach activities to connect with authorized participants, while also providing clinical services as needed.

Responsibilities

  • Providing engagement and case management support including home-based visitations and other services within the Philadelphia community.
  • Interacting with members of the multidisciplinary team to ensure that participants receive individualized services designed to help them maintain their permanent housing.
  • Conducting and coordinating outreach activities to locate and engage TCMU authorized participants.
  • Providing clinical services to participants in their natural environment or assisting them in accessing mainstream clinical services.
  • Working with participants to develop, implement, and monitor their personal goal plans to stabilize and retain housing.

Requirements

  • Bachelor's degree with concentration in sociology, social work, psychology, gerontology, anthropology, criminal justice, theology, nursing, counseling, or education.
  • Ability to communicate effectively both verbally and in writing with all levels of the behavioral health system.
  • Skills to establish supportive trusting relationships with individuals with severe and persistent mental illnesses.
  • Ability to work independently and as part of a focused team.
  • Working knowledge of Microsoft Outlook, MS Word, and Excel.
  • Ability to maintain daily productivity standards.
  • Valid driver's license with an acceptable driving record.
