About the position

The Housing First Case Manager at Lt. Col. Matt Urban Human Services Center of WNY is responsible for providing comprehensive case management services to vulnerable populations, particularly those experiencing homelessness or related challenges. This role involves assessing client needs, developing individualized service plans, and facilitating access to necessary resources and support systems to promote self-sufficiency and housing stability.

Responsibilities

  • Understand and implement the Housing First model, including documentation of chronic homelessness.
  • Conduct thorough assessments of clients' social history, treatment histories, and housing preferences.
  • Evaluate clients for eligibility for SPMI (mental health) services.
  • Assess independent living skills and work with clients to enhance self-sufficiency.
  • Complete and maintain Independent Services Plans (ISP) with regular updates on client progress.
  • Ensure compliance with client confidentiality laws and manage release of information.
  • Link clients to medical and community supports such as SSD, SSI, SNAP, and employment services.
  • Conduct monthly home visits and maintain regular contact with clients.
  • Utilize HMIS/BAS-Net for data entry and maintain both electronic and paper client files.
  • Prepare for audits and maintain accurate records of client activities and services provided.
  • Conduct annual housing unit inspections and address maintenance issues with landlords.
  • Serve as a liaison between clients and landlords, ensuring effective communication and support.
  • Participate in workshops, trainings, and meetings related to homeless services and community support.
  • Prepare timely reports and case notes within 72 hours of client interactions.

Requirements

  • Bachelor's Degree in Social Work, Human Services, Counseling, or equivalent experience.
  • 2+ years of experience working with vulnerable populations, including those with homelessness, chronic illness, or mental health issues.
  • Valid NYS Driver's License and access to an insured vehicle for client services.
  • Knowledge of HIPAA regulations and case management best practices.

Nice-to-haves

  • Experience with the Housing First model and related documentation processes.
  • Familiarity with community resources and services available in Erie County.
