About the position

The Case Manager for Housing at Avivo plays a crucial role in supporting individuals experiencing homelessness by providing resources and assistance to achieve housing stability. This position is part of Avivo's Ending Homelessness division, which focuses on culturally responsive and trauma-informed service delivery. The Case Manager will engage with clients, assess their needs, and develop personalized goal plans to facilitate their transition to stable housing.

Responsibilities

  • Engage and build rapport with clients experiencing homelessness.
  • Assess eligibility for programs and services and connect clients accordingly.
  • Develop individualized Goal Plans for clients.
  • Make appropriate referrals and monitor client progress, ensuring follow-up.
  • Utilize creative approaches to meet clients in person and address their needs regularly.
  • Consult on safety-related concerns, mandated reporting issues, and client complaints.
  • Provide housing-specific services and build relationships with landlords.
  • Complete paperwork, documentation, and compliance tasks on time.
  • Develop Crisis Plans and conduct crisis interventions as needed.
  • Ensure services are culturally responsive and utilize best practices.

Requirements

  • High School Diploma or GED required.
  • One or more years of experience working with single adults experiencing homelessness or similar experience.
  • Valid driver's license and access to a personal vehicle with proof of valid insurance and a clean driving record.
  • Ability to pass a DHS background check.

Nice-to-haves

  • Openness to Harm Reduction and Housing First principles.
  • Basic knowledge of mental health, chemical health, community resources, trauma, institutional racism, gender issues, and tenants' rights.
  • Knowledge of community resources or where to find them.
  • Ability to proactively advocate for clients.
  • Ability to work with individuals from diverse backgrounds.
  • Basic knowledge of de-escalation and mediation techniques.
  • Flexibility to adapt to various situations.
