About the position

The Housing Stabilization Coordinator (HSC) at Friendship Shelter is responsible for assisting formerly homeless individuals who are newly housed through the Permanent Supportive Housing program. The HSC will develop individualized service plans to promote housing retention, client stabilization, and independent living skills, ensuring that clients receive the necessary support to maintain their housing and improve their quality of life.

Responsibilities

  • Assess each client's needs using agency-provided tools and create an individual stabilization plan (ISP).
  • Provide ongoing case management through regular appointments and follow-ups.
  • Assist clients in accessing resources to increase housing stability, including employment and income benefits.
  • Conduct regular home visits and accompany clients to appointments related to housing stability.
  • Respond to crises to prevent returns to homelessness, promoting dignity and respect in all interactions.
  • Provide instruction and assistance to enhance clients' independent living skills.
  • Quickly resolve lease violations and provide accurate follow-up with supervisors.
  • Maintain accurate case notes and report client data as required.
  • Participate in case reviews, tenant meetings, and staff meetings.
  • Maintain positive relationships with staff, partnering agencies, and community volunteers.

Requirements

  • Experience working with individuals with severe and persistent mental illness or substance use disorder.
  • Familiarity with local Orange County social service systems such as Medi-Cal and food stamps.
  • Direct service experience preferred.
  • Reliable transportation and valid driver's license.
  • Excellent communication, problem-solving, and organizational skills.
  • Ability to work in a Housing First, Trauma Informed, and Harm Reduction setting.

Nice-to-haves

  • Experience navigating social service or behavioral health providers.
  • Basic computer skills including Microsoft Office applications.
