About the position
New River Valley Community Services is seeking a Case Manager I or II at New Horizons! The Case Manager provides case management for clients of the crisis stabilization program, including clients with mental illness and co-occurring substance abuse. The Case Manager coordinates services, resources needed by the client during residential services, and monitors outcomes up to 30 days post discharge. This role also provides coordination for discharge services with agency and community resources. In this role, you will coordinate with Special Justices, program psychiatric staff, and Independent Evaluators for commitment hearings held on-site. Requirements
- For Case Manager I: High School Diploma with at least one year of experience in a setting working with individuals with mental illness, substance use disorders and/or developmental disabilities.
- For Case Manager II: Graduation from an accredited college or university with a Bachelor's Degree in Social Work, Psychology or other related field.
- Registration as a QMHP or trainee is required for Case Manager II.
- Must maintain QMHP status and all required trainings/CEUs throughout employment.
Nice-to-haves
- Bachelor's Degree in Education, Social Work, Psychology or Human Services related field for Case Manager I.
- Experience in a setting working with individuals with mental illness, substance use disorders and/or developmental disabilities for Case Manager I.
- Additional compensation for speaking multiple languages.
